Dear Safety Net Dental Care Providers / Stakeholders:



Would you like to have an opportunity to shape the training and technical
assistance services available to Safety Net Dental Clinics (SNDCs) in Ohio?
Please take a few minutes to complete this survey and share your input as we
explore strategies to support SNDC staff.

Like you, we care deeply about accessible, sustainable oral health services
for all Ohioans, especially the most vulnerable. Like you, we see federal,
state and local budget cuts and shifting priorities and know that providing
access to care in a sustainable way will continue to be a major challenge.
We have to find ways to be more efficient and effective in how we serve
those in need.



The Oral Health Capacity Building Project, initiated by the HealthPath
Foundation of Ohio, in collaboration with the Ohio Department of Health,
Osteopathic Heritage Foundations, and the Sisters of Charity Foundation of
Canton, is committed to finding ways to work with others who care about
sustainable access to oral health care for the vulnerable.  As funders we
know we can’t do it alone.  We also know we can learn a lot from the
considerable professional and personal experience among those of you who
work in or work with SNDCs.



Please complete the survey no later than Friday, April 22.  Your input will
help us provide funding support to training and technical assistance
services that best meet your needs.
